This document is a summary of Cleveland Clinic’s benefit offerings. Some of the

highlights include:

 

-          Employee Health Plan / Prescription Drug Benefits / Dental Coverage / Vision

Care Benefits (pages 5-13)

o    Comprehensive Health Plan with no deductibles, no copays or coinsurance for primary care

Visits (includes Express Care, Express Care Online app and select real-time virtual visits).

o    New to 2022 Health Plan

§  Coverage for LASIK surgery

§  Infertility treatment – lifetime maximum of $15,000

o    Please refer to the EHP Plan (EHP Plus is designed for caregivers who live outside the

Cleveland area and need access to a national network of providers). Review

§  There are opportunities to reduce your health plan premiums each year by participating

in our Healthy Choice Program (page 18 of 2022 OH Employee My Pay + Benefits

Highlights).  For more detailed information on the Health Choice Program, please CLICK HERE

 

-          Short and Long Term Disability (page 14) Eligible to caregivers .9 FTE and above

 

-          Maternity and Parental Leave (page 14) Eligible for full-time caregivers

o    100% pay for 8 weeks of maternity leave for mothers following childbirth

o    100% pay for 4 weeks of parental leave for both parents following the birth or adoption of a child

 

-          Retirement programs (page 15). Caregivers become fully vested in these programs after 3

 years of service.

o    Cleveland Clinic offers an Investment Pension Plan (IPP). Only Cleveland Clinic contributes to this

Plan.  Contributions increase with years of service, starting at 2.5% of your salary and maxing out

at 5.5%. Caregivers become fully vested after 3 years of service.

o    Cleveland Clinic offers a 403(b) Savings & Investment Plan (SIP). Cleveland matches $.50 for

every $1 of caregiver contributions, with a maximum matching contribution of 3% each pay

period.  To receive the maximum matching contribution caregivers should set their contributions

to at least 6%.

o    Healthcare FSA

o    Dependent Care FSA

 

-          Paid Time Off (PTO) (page 18)

o    Advanced practice providers start off with 28 days of PTO (includes 6 Cleveland Clinic observed

holidays). These hours are accumulated per pay period and you are eligible to start using your

PTO after your 90 day new hire period. You can carry unused hours into a vacation bank each

year or use PTO towards paying for your benefits during open enrollment each fall.

o    In addition, Advanced Practice Providers are eligible for 5 days of CME per year based on

departmental approval
