Additional services of this specialized laboratory include the processing of sperm for intrauterine insemination; for artificial insemination; and sperm cryopreservation for patients prior to chemotherapy, radiation therapy, retroperitoneal lymph node dissection or vasectomy.
The Cleveland Clinic Andrology Laboratory and Reproductive Tissue Bank has provided therapeutic sperm banking services since 1980 - longer than any other facility in Ohio. The laboratory staff is trained in the latest, most effective freezing/ cryopreservation techniques, storage and recovery techniques, and work under the direction of a sperm cryopreservation specialist. The highest standards are used in accurately labeling and identifying specimens. The Cleveland Clinic laboratory is CLIA - certified and is also accredited by the American Society for Reproductive Medicine and the College of American Pathologists, as well as inspected/registered with FDA.
Semen specimens are collected on-site at the Cleveland Clinic Andrology Laboratory in a private collection room. We recommend collecting the sample at the laboratory, because the sample's freshness affects fertility. Within minutes after ejaculation, the number of live sperm cells and their activity begins to drop off rapidly.
Every specimen is carefully labeled and coded to ensure accurate identification and confidentiality. The Cleveland Clinic Andrology Laboratory and Sperm Bank has a system of checks & balances to ensure that specimens are properly identified at the time of storage, during storage and at the time of release. A label is printed that has patient name, medical record number, an internal control number and the date. Semen may be stored indefinitely at the final temperature of -196° C. A man may store his sperm with the Cleveland Clinic Andrology Laboratory and Sperm Bank for an open-ended period, with an annual storage fee.
The Cleveland Clinic's Reproductive Tissue Bank may be used to store frozen semen specimens for male oncology patients of reproductive age who have been diagnosed with testicular carcinoma, Hodgkin's disease / lymphoma, or any other disease which requires chemotherapy, radiation therapy, or transurethral resection.
Frozen semen specimens may be stored prior to a vasectomy/military deployment to allow the participant the option of future insemination.

The NextGen Home Sperm Banking Kit is a first-of-its kind product designed for men who wish to preserve their fertility for a variety of reasons. This can include
1) men with cancer who are about to undergo treatment (surgery, chemotherapy, radiation therapy, which can render them infertile;
2) men with underlying subfertility;
3) men who are about to under a vasectomy but who do not want to totally rule out the option of fathering a biological child in the future and
4) military personnel going on long-term deployment whose partners may need a sperm sample for an assisted reproductive procedure while they are gone.
With this kit, patient collects a semen sample in the privacy of their own homes, place the sample into a vial containing preservative/transport media, and ship the samples overnight to the Cleveland Clinic Andrology Laboratory and Reproductive Tissue Bank for storage. All samples undergo a standard analysis that includes sperm count, motility and viability. These results are reported back to the patient within 2-3 days. Once stored, these samples can then be used in assisted reproductive procedures such as intra-uterine insemination, in vitro fertilization and intracytoplasmic sperm injection.
This novel approach is advantageous in that it spares patients the need to spend time and money traveling to a remote clinical site as well as embarrassment and stress over having to produce a sample on demand in an unfamiliar setting.
