What is the new MyChart Caregiver service?

 

·         The new MyChart for Caregivers services gives parents or legal guardians of children (age 12 years and younger) who receive their primary care from a Cleveland Clinic Pediatrician located at The Cleveland Clinic’s main downtown campus access to portions of their child’s medical records.

 

PLEASE NOTE:  Presently, the new MyChart Caregiver service is ONLY available for patients who receive their primary care from a Cleveland Clinic Pediatrician located at The Cleveland Clinic’s main downtown campus.  MyChart Caregiver will become available to the patients of Cleveland Clinic Pediatricians seen at Cleveland Clinic Family Health Center locations in the coming months.

 

Do I need any special computer equipment to use MyChart or the MyChart Caregiver service?

 

·         No. As long as you have an email account and access to a computer connected to the Internet you may access MyChart anytime, day or night.

How do I sign up for MyChart Caregiver?


The MyChart Caregiver service requires the requestor to have their own MyChart account. There are two ways to sign up for MyChart Caregiver:

 

1.   At the physician’s office

 

·         If you are the parent of a minor child, and you have been treated at The Cleveland Clinic’s downtown campus or at any Cleveland Clinic Family Health Center (meaning that you have a Cleveland Clinic Medical Record Number), you may sign up for a MyChart Caregiver account at the office of the Cleveland Clinic Pediatrician treating your child. If you do not have your own MyChart account, an account will be created for you at this time. You will be required to show a photo ID as proof of your relationship to your child.

 

2.   At the Cleveland Clinic Department of Health Data Services

 

For all other caregiver relationships, including legal guardian or Durable Power of Attorney for Healthcare for a dependent loved one, and/or if you have never been a patient at The Cleveland Clinic or a Cleveland Clinic Family Health Center (you do not have a Cleveland Clinic Medical Record Number):

 

·         Obtain a MyChart “Proxy Access Request” form at the desk of the physician treating your loved one, or online at www.clevelandclinic.org/mychartinfo
(select and print the MyChart “Proxy Access Request” form).

 

·         Complete the form and supply the requested documents pertaining to proof of your relationship to the patient for whom you are requesting MyChart Caregiver access.

 

·         Fax or mail all completed materials to:

Following Health Data Services Department processing, you will receive a letter in the mail containing the account information.

 

Does it cost anything to sign up for MyChart Caregiver?

 

No. Both MyChart and the MyChart Caregiver service are offered free of cost to Cleveland Clinic patients.
