What is eCleveland Clinic MyChart ®?
MyChart: Your Personal Health Connection™ is a secure, on-line health management tool that is used by tens of thousands of Cleveland Clinic patients to:

  • Review past appointments and physician instructions
  • Receive laboratory and other test results released by their physician(s)
  • Request new appointments and prescription renewals
  • Access reliable health information about a broad range of topics of personal interest

Who is eligible for a personal MyChart account?
MyChart is presently available to Cleveland Clinic patients who see a primary care physician (Internal Medicine / Family Practice) at The Cleveland Clinic’s main downtown campus or at any Cleveland Clinic Family Health Center location.

MyChart is also available to patientswho see a physician in the following Speciality Departments at the Cleveland Clinic’s main downtown campus:

Rheumatology, Immunology, Executive Health, Infectious Disease, Endocrinology, Urology, Psychiatry, Nephrology/Hypertension, Obstetrics and Gynecology.

Can I get a MyChart account for a loved one for whom I am responsible?

Yes! If you are the parent or legal guardian of a child who received their primary care from a physician in the Department of Pediatrics at The Cleveland Clinic’s main downtown campus, or at the Lorain or Wooster Family Health Centers, or if you are responsible for an adult dependent loved one (Durable Power of Attorney for Healthcare), our new MyChart ● Caregiver service is perfect for you.

With the new MyChart ● Caregiver service you will have the same convenient, 24 hour online access to information and eHealth services for your dependent loved one as you enjoy with your own MyChart account. Do I need any special computer equipment to use MyChart, or MyChart Caregiver?

No. All you need is access to a computer, an email account, and an Internet connection.

How do I sign up for MyChart?
You may establish your own personal MyChart account in either of two convenient ways:

  • The next time you visit your physician, ask your physician service representative to generate a one-time-use Access Code for your personal MyChart account.
  • Click the "On-Line MyChart Sign-Up" link in the menu bar location on the right side of this screen. Complete the on-line MyChart sign-up form by entering the requested information. (Please note that the information you provide is confidential and it will be processed through our secure Web site.) Your request will be reviewed and your information verified; then a special, one-time-use MyChart Access Code will be mailed to your home via the US Postal Service. Once you receive your one-time-use Access Code, please use it immediately to activate your MyChart Account, as the Code is only valid for 60 days.

 How can I establish myself as a Cleveland Clinic patient and begin using my own personal MyChart account?

If you are not presently a Cleveland Clinic patient but would like to establish yourself with a Cleveland Clinic Primary Care physician (General Internal Medicine / Family Medicine), please call 888/MED-WELL (888/633-9355). An appointment specialist will assist you in identifying a Primary Care physician at a Cleveland Clinic location near your home. You may also visit www.clevelandclinic.org/myappointment to request an appointment with a General Internal Medicine or Family Medicine practitioner at a location of your choice. You may establish your personal MyChart account the first time you visit your new Primary Care physician's office.

 Can I email my Cleveland Clinic physician through MyChart?
MyChart is not an email system and we do not recommend sending any confidential information through standard Internet email. When you activate your personal MyChart account, you will be asked to provide your email address so that you can receive email notification to log on to your secure MyChart account whenever new information has been made available. Any confidential patient information transmitted from your Cleveland Clinic healthcare team will be located in secure messages that you will see only after you have logged into your secure MyChart account.

Will my Cleveland Clinic physician contact me through MyChart?
Yes. When your physician releases selected test results, a message may be included with your results to help you better understand the significance of the information you receive. Your physician may also send you secure messages through your MyChart account.

Does every Cleveland Clinic patient automatically have MyChart?
No. Though we strongly encourage you to sign up for MyChart so that you may benefit from its many convenient uses, a MyChart account will only be established for you at your specific request. We respect your personal preference regarding communication with your Cleveland Clinic physician, and we do not require anyone to become a member of the MyChart community.

Is there a monthly charge for MyChart?
NO! There is no charge to Cleveland Clinic patients for currently available eCleveland Clinic MyChart services.
