Staff physicians at The Cleveland Clinic are consistently recognized

In addition, many Cleveland Clinic physicians lead research
trials and collaborate with The Cleveland Clinic’s basic scientists to
bring the latest research to patients as quickly as possible.
Is The Cleveland Clinic MyConsult program available to anyone?
The Cleveland Clinic online medical second opinion program is available
in most locations. In some states, to comply with state licensing requirements,
the patient’s physician will need to sign a “request for consultation”
form. The program is not available in a few states that do
not allow for remote consultation. The patient will be notifi ed if the
service is not available.

service available in every state?
We are not allowed to offer online medical second opinions to patients
whose mailing address is in one of the following locations: Guam,
North Dakota, and California. However, if you wish to obtain a second opinion
from a Cleveland Clinic physician we will be happy to assist you in
making an appointment to be seen at Cleveland Clinic. Call us at

Can I request a Cleveland Clinic MyConsult Online Medical
Second Opinion for someone else?
Yes, if you have the patient’s permission. The patient must be over 18
and capable of providing written consent. Print the Cleveland Clinic
proxy form at www.eclevelandclinic.org/static-html/proxyform.html,
and have the patient complete and sign and date the form.
If the patient is under age 18 and you are the child’s parent or legal
guardian you do not need to print and complete a proxy form. If the
patient is under age 18 and you are not the patient’s parent or legal
guardian, you must have the patient’s parent or legal guardian,
complete a proxy form to proceed with the online medical second
opinion. Print the proxy form.
How much does a Cleveland Clinic MyConsult Online Medical
Second Opinion cost?
The cost of a Cleveland Clinic MyConsult Online Medical Second
Opinion is $565, payable in US funds at the time of the request.
If a pathology review (reading) is required to complete your MyConsult
Online Medical Second Opinion (as is the case in most cancer-related
diagnoses) there will be an additional charge of $180 for that review
for a total of $745 for the consult and reading.
Cleveland Clinic is unable to fi le a claim for online medical second
opinion services with any insurance company including Medicare. If

Is a Cleveland Clinic MyConsult Online Medical Second Opinion
covered by my insurance?
The Cleveland Clinic MyConsult Online Medical Second Opinion service
is not covered by insurance programs.
Many employers have contracted with Cleveland Clinic to provide the
MyConsult program as part of their employee benefi t plan. If you are
not sure if your company provides coverage, call the Cleveland Clinic

What equipment do I need?
If you have a computer that can access the internet, you can probably
use our site.
Specifically, we use 128-bit encryption to secure your data — this
ensures that only you and the staff at Cleveland Clinic have access
to your data.
Can I cancel my Online Medical Second Opinion request?
You may cancel your online medical second opinion request at any time
while you are fi lling out the forms online. Simply click on the “cancel”
button at the end of any section. Your information will not be saved or
sent to The Cleveland Clinic. Once you have authorized us to proceed
with your consult (upon completion of the online steps) and our staff is
working to complete the consult you may not cancel the request.
Please let us know if you have extenuating circumstances.
How long will it take to receive my opinion?
Your opinion will be reviewed by our specialist as quickly as possible.
Our goal is to complete your consultation within ten (10) business days
from the time we have received all of the requested medical information
and supporting documents (International shipments take at least three
days, and as long as fi ve days, for materials to arrive at our offi ce). It
will take approximately two days longer if we are reading pathology
slides.
Do I need my local doctor’s permission to request a Cleveland Clinic
MyConsult Online Medical Second Opinion?
You may contact Cleveland Clinic directly to request your second
opinion. However, you will need to contact your local doctor’s office
to obtain medical information or tests that are necessary for the
Cleveland Clinic physician to complete your second opinion. In some
states, to comply with state law, you may need to have your primary
or specialty physician submit a “Physician Consent” form to proceed
with the consult. You will be notifi ed during the consult online
request submission if this pertains to you.
How do I obtain copies of my medical records or original medical tests?
To obtain copies of your medical records or tests contact your
physician’s offi ce or the facility where the tests were performed. You will need to supply the offi ce or
facility with a written request for the information, including medical
records (paperwork fi les), films (X-rays, MRI, etc.) or pathology slides.
Specify that you would like the record to be released to you,
not The Cleveland Clinic. It may require 7-14 days for your physician or
facility to complete your request.
Where do I send my materials?
For your convenience, you may use the mailing label provided in
the “Mailing Instructions” step of the Request Opinion process.
The materials should be sent to Cleveland Clinic using a service that
requires a signature from the recipient, such as Federal Express,

Will my Cleveland Clinic doctor perform a physical exam?
No. Your Cleveland Clinic doctor will carefully review the medical
information that you provide, including your local doctor’s fi ndings
during your physical exam. The Cleveland Clinic doctor also will
provide a thorough review and assessment of your medical tests
or studies.
