Cleveland Clinic Heart Failure Disease Management Program
The Cleveland Clinic Heart Failure Disease Management Program understands heart failure and heart failure treatment. A certified nurse practitioner provides direct care and coordinates heart failure treatment with your Cleveland Clinic physician. In this program, we will:
  • Ensure that your condition is fully diagnosed
  • See that the treatment and medication plan are appropriate to your specific condition, according to national heart failure treatment guidelines
  • Improve and maintain your optimum quality of life, and minimize the need for hospitalization and emergency department care
  • Help with lifestyle changes to understand and mange your condition
  • Ensure that heart failure management blends with management of other health conditions
  • Provide answers to your questions and provide heart failure management advice 24 hours a day, seven days a week.

What does the Heart Failure Disease Management Program Offer?

  • Complete cardiac assessment and treatment adjustments to assure optimum cardiac function and relieve or prevent symptoms
  • Lab assessment
  • Expert evaluation of factors that may worsen or magnify your condition
  • Expert recommendation on changes in lifestyle, education and individual need to more effectively manage symptoms and prevent hospitalization
  • An educational book on heart failure will be provided to you, with materials on identifying the signs that determine when to visit your physician or seek emergency care; medication; salt intake reduction; dealing with stress and depression; weight management to prevent congestion and swelling; and information about routine tests and treatments. These materials will help you learn more about your condition and will help you better mange your condition at home
  • Routine follow-up telephone calls, even if you do not have symptoms and do not need an office visit
  • Support group, Nutrition Services, Social Work and Cardiac Rehabilitation Program services
  • 24 hour-a-day, 7 days-a-week management advice available to answer your questions
  • Access to other heart failure programs and services, such as a Heart Failure Home Care Program, Palliative Care Heart Failure Program, or our Heart Failure Research Program.

Where is the program located?

The Heart Failure Disease Management Program is located at the Cleveland Clinic's main campus, Cardiovascular Medicine Department (Desk F25).

How do you get into the program?

Ask your Cleveland Clinic doctor. If you do not have a Cleveland Clinic doctor, we can provide you with an internist or cardiologist (heart failure specialist) to get you started.
