Where is the Cleveland
Clinic Heart and Vascular Institute located?
The Heart and Vascular Institute is
located in Cleveland, Ohio, just east of downtown Cleveland. The Cleveland
Clinic medical campus comprises buildings that are coded with letters
of the alphabet. The Cardiology and Cardiothoracic Surgery practices primarily occupies buildings F and G. Vascular Medicine and Vascular Surgery is located in Building S. Preventive Cardiology is in the Walker Center, Building C.

How can I get a second opinion?
If
you would like to come to the Cleveland Clinic for a second opinion, please
call the Heart Center Information and Resource Nurse, toll-free at 866-289-6911.
She will help you make an appointment to see the correct specialist to
evaluate your condition. You will need to bring your prior medical records.

How do you transfer a patient to the Cleveland Clinic Foundation Heart Center?

If you would like a family member transferred from a hospital to the Cleveland Clinic Heart Center, please have the attending physician at the current hospital follow the following directions:
Cardiology patients: Call Patient Access Services at 216/444-8302 or 800/553-5056. This is a direct-access, 24-hour phone line for physicians with patients who need to be transferred from their hospitals to our facility.
Surgery patients: Call our Nurse Practice Managers at 216/444-2200, beeper 25190 or 800/223-2273, beeper 25190 (outside of the Cleveland area) between the hours of 7am and 5pm (eastern standard time), Monday through Friday. At other times, you may call Patient Access Services at 216/444-8302 or 800/553-5056
Does my insurance
cover care at the Cleveland Clinic Heart Center?
The Cleveland Clinic participates with most major medical insurance programs. What should I do
if I have Billing or insurance related problems?

How do I get a copy of my medical records from the Cleveland Clinic?
Click here to get a copy of our Medical Release Authorization Form. You will need to sign and fax or mail this form back to Health Data Services. The address and phone numbers are located on the form. Normal processing time is 7-10 business days, depending on the availability of the medical record.
