WEIGHT WATCHERS APPLICATION PROCESS EFFECTIVE JUNE 1, 2011

• Go to https://clevelandclinicwwapp.com and create an account.  (Don’t forget to retain your login and password for future transactions.)
• Provide your EHP ID number located on your healthplan card..  Make sure you enter “EHP” in front of the number. (Florida members – use your ten digit ID from your UMR ID card).  You must use the ID number of the person that is signing up.   If your dependent is signing up, use his/her ID number.
• Enter all remaining information as requested.  NOTE:  The meeting location number is only required for At Work meetings.
• Upon completion of the electronic signature, hit CONTINUE.  A reference number will appear online.  NOTE:  Print a copy of your reference sheet to include with a copy of your weight book at the END of the session.

Next, call the Weight Watchers toll-free number at 1-866-614-9130 and give them the reference number.  This will show you have completed the application.

• Weight Watchers will give you a confirmation number (Online enrollees will receive an Access Code)..  You will need to give your confirmation number to your session leader at the first meeting.  The Access Code will be needed to access the Online program.

To access your account in the future, go to https://clevelandclinicwwapp.com and click on the “sign in” button. Sign in with the login and password you created when you registered.

If You Don’t Have Access to a Computer

You can still sign up with a paper application.   After completing the application), mail it to the EHP at the address on the form.  You can obtain an application by calling the EHP 216-448-0800 or toll-free at 1-866-811-4352.  It is also available on our web site at www.clevelandclinic.org/healthplan. The EHP will enter your application online and contact you with a reference number so you can call Weight Watchers for your confirmation number.  NOTE:   Please be aware that signing up this way takes longer because of the time needed for mail delivery and data entry.  We strongly recommend doing the online application.

How to Submit Weight Watchers Log Book

You will be able to upload your log book from your session online at https://www.clevelandclinicwwapp.com.   Along with your book, include a copy of your reference sheet that you printed when you signed up.  Make sure your first and last name, as well as your EHP ID number is on the document.

For each session you join, a new application must be completed.  You will not be able to join a new session until your book from the previous session is received and entered into the system.  It takes five business days from date the book is received for entry.
