Take a Few Steps Now - and Benefit in 2015!

How Healthy Choice Works

 

Each year in early January, caregivers, spouses and domestic partners covered by the health plan will receive a personalized letter outlining the information the plan has on record and if additional information is required. The letter will also describe what the plan member needs to do over the next nine months to earn the best premium for the following year.

 

Below are the steps to take, depending on the type of letter you receive:

You will receive one of these:

 

Missing Information Letter

Chronic Condition Letter

“Healthy” Letter

What it says:

You need to submit more health information.

What it says:

You’ve been diagnosed with high cholesterol, high blood pressure, weight management, diabetes, asthma or tobacco use*.

What it says:

You are healthy.

(Not diagnosed with one of the chronic conditions listed at the left)

What you need to do:

 - Schedule a health visit to have the Health Visit Report Form completed and submitted to the health plan

AND

 - After your visit, if you are healthy or receive one of the 6 diagnoses, follow the steps in one of the boxes at the right.

 

What you need to do:

Join the appropriate Coordinated Care Program(s) and meet its (their) requirements for six months by Sept. 30, 2014.

What you need to do:

 - Join an EHP Physical Activity program and go 10 times a month for 6 months. Click here for information on fitness programs.

 

OR

 - Participate in Shape Up & Go! and wear a Pebble monitor to track at least 100,000 steps or 600 physical activity minutes a month for six months by Sept. 30, 2014.  Click here for information on the Pebble.

 

 

Important Healthy Choice Deadlines

March 31, 2014: Final day to join a Cleveland Clinic-sponsored fitness center, Curves or a Coordinated Care Program—or get your Pebble registered and start participating in Shape Up & Go!  The deadline for appeals of condition diagnosis or premium level is March 31st of each year.

Sept. 30, 2014: Deadline for meeting Healthy Choice requirements; health visit report form and participation data must be submitted

*How is the Privacy of EHP Members Protected in the Rollout of the Healthy Choice Program?

Under HIPAA, EHP like other health insurers, is permitted to access health data for the purposes of claims payment, health program development and treatment coverage. The letters that were mailed to EHP member employees about next steps to participate in Healthy Choice are confidential communications between EHP and its members and contained EHP data generated using claims payment. wellness program and limited medical information. As with any of our healthcare plans and programs, plan member privacy is protected in full compliance with HIPAA.
