Information about the 2012 Healthy Choice Program is included in the Open Enrollment information.  Packets will be mailed to employee homes the week of October 17, 2011.

**A $100 incentive will be offered to members who participate in the EHP Physical Activity Program

for 10 times per month for ten months, starting no later than March 1, 2011. 

The $100 incentive is only for fitness center participation (Curves or Cleveland

Clinic fitness centers) and is only available to the employee members of EHP.

The $100 incentive does not apply to Shape Up & Go!

Proof of Participation Is Required

Participation in the Healthy Choice Program requires all participants to provide documented progress from one of the following:  

1. EHP Medical Management

2. Your physician

3. EHP Wellness Programs, or Shape Up & Go!

• If you are healthy, you can earn the 17% EHP premium rebate for 2011 by either participating in one of the EHP Physical Activity Programs ten times a month for eight months by October 31, 2011 or participating in two 12-week Shape Up & Go! sessions in 2011.  
• If you have one or more of the six diagnoses the program focuses on, you can earn the 17% premium rebate for 2011 by joining the appropriate Coordinated Care Program(s) and meeting all program goals within eight months as of Oct. 14, 2011.
