Reminder: EHP premium structure is changing in 2015

Healthy Choice Program 2014

The 2014 Healthy Choice program available to Employee Health Plan (EHP) members has changed in two ways:

-  Healthy Choice is now offered to spouses and domestic partners - giving them the same great opportunity to take charge of their health.   EHP Health Visit Form (click here)

- Health Plan members who participate in Shape Up and Go! to qualify for Healthy Choice will need to wear an electronic Pebble device that measures physical activity in order to earn the health plan's best premium opportunities.  Click here for more information about the Pebble.

EHP Weight Management Programs

EHP offers programs that include Weight Watchers, Curves Complete, and Cleveland Clinic weight management programs that may be referred by your nurse case manager in Coordinated Care and can help you qualify for the Healthy Choice program.  Click here for more information.
