Our Voice: Healthcare Partners Application Form

Our Voice: Healthcare Partners are patients, family members and/or support persons who work together with Cleveland Clinic Caregivers to transform healthcare and support our vision of Patients First and providing relationship-centered care.

We are seeking candidates to participate in the Our Voice Council, a program that invites you to contribute to transform healthcare by improve the patient experience, help design physical space, participate on specific committees focused on improving quality, patient safety, population health and ethics, speaking about your experiences to staff members at the Cleveland Clinic, participate on research projects, and/or respond to surveys or electronic feedback. Involvement may occur throughout the Cleveland Clinic including main campus, regional hospitals, outpatient family health centers and primary care practices.

Please complete this form to be considered as a candidate for the Our Voice: Healthcare Partners Council. You may participate as a patient, family member, or support person to a patient.

All information contained on this form is considered confidential and is intended for use by the Cleveland Clinic Our Voice: Healthcare Partners selection committee only.

You may be contacted upon receipt of this application form to participate in a phone or in-person interview with someone from the Office of Patient Experience. The Our Voice: Healthcare Partners are considered representatives of the Cleveland Clinic, who volunteer their time to participate. If selected you will be asked to become a member of the Cleveland Clinic Volunteer Services Department and this involves completing the Cleveland Clinic on-line onboarding process, a background check, TB test and participation in an orientation session.
