Clinical Relevance Of Diagnostic Wavefront Imaging

Purpose: Visual distortion and loss of best spectacle-corrected visual acuity is an undesirable finding associated with refractive surgery. Wavefront imaging offers a practical diagnostic solution.

Methods: Using the Alcon LADARWave in a U.S. FDA investigational trial, 60 eyes were examined before and after LASIK and the creation of a flap only to determine the clinical value of this device in diagnosing aberrations. Both the Alcon SKBM and Moria M2 microkeratomes were used in conjunction with the Alcon LADARVision 4000 laser during the study.

Results: The magnitude of aberrations induced following myopic LASIK included a mean increase in spherical aberration by 115%, coma by 46%, and trifoil and other terms by 58% in 34 eyes at 4 months. Hyperopia LASIK tended to demonstrate a decrease or negative change in spherical aberration. Twenty flap-only eyes also demonstrated minimal aberration changes. In patients desiring enhancement, the magnitude and axis of astigmatism was correlated with the manifest and cycloplegic refraction and topography to refine further correction.

Conclusion: Wavefront imaging with the LADARWave is valuable in quantifying and displaying the components of refractive error and higher order aberrations in post op LASIK and flap-only eyes. It provides powerful diagnostic information for an active refractive surgery practice.
