If your rotation is approved, you will receive a letter of appointment from the Graduate Medical Education Department. The morning of your orientation please park in the visitors parking garage on East 100th Street, the parking office will validate your ticket when parking arrangements are made. You will need to report to GME (2nd Floor Room NA2-55, Education Wing of Lerner Research Institute) on your start date at 8:30 a.m., please be on time as other trainees attend this session as well. If you are not here on time, you will not be able to start your rotation until the following day. Please DO NOT report to the department until you have been oriented.

• Documentation of Varicella and Rubella titers or proof of MMR immunization
• Recent documentation of TST test or recent chest x-ray (<<1 year) if known to be TST positive
• Proof of hepatitis B immunity (serology)
• Proof of Bloodborne Pathogen Training (<<1 year)

As a part of your orientation, you will be required to attend a session on Bloodborne Pathogens, HIPPA, and environmental safety (unless you have provided proof of training within the state of Ohio during the past year).

• Report To ID Badge Department (Area W, Level A): All visitors are required to obtain a temporary picture badge from the ID Badge Center. Each visitor pays a $10 deposit for the badge, which will be refunded when the ID Badge is returned at the end of you rotation. The ID Badge Department is open 8:00 a.m. - 4:15 p.m.
• Parking (Area W, Level A: There is a $50 charge per month for garage parking. The Parking Office is open 7:00 a.m. – 4:45 p.m. GME does not pay for your parking
• Uniform Room (Basement Area H): White lab coats can be borrowed from the uniform room and replaced when soiled. The OR Uniform Room will issue uniforms worn in the operating rooms. The Uniform Room is open 7:00 a.m. – 4:00 a.m. on Mondays and 7:00 am - 9:00 am and 2:00 pm - 4:00 pm Tuesday through Friday.
• Pagers (Basement Of The Crile Building, Room Ab118B): Pagers are issued to visiting residents/fellows. You will need to sign a financial responsibility form prior to obtaining a pager; there will be a charge of $60.00 for lost pagers.
