How to Refer a Patient to a Cleveland Clinic Physician

Call the Affiliate Hotline 24 hours a day at 216-444-8302 or 1-800-553-5056. For Admits or Hospital Transfers, press option 1, for Appointments, press option 2.

Foreign patient referrals can be facilitated by the International Center at 216/444-6404

If your patient requires hospitalization, please contact the physician to whom you will be referring to discuss the case, or call the Affiliate Hotline to facilitate the process. Emergency transportation or hospital transfer can be arranged if needed. To avoid duplication of x-rays or laboratory tests, please send ahead or have your patient bring to his/her appointment any laboratory test results and x-rays.

Referral for Laboratory Testing

Patient Testing Information, 216/444-3000, 800/CCF-CARE (223-2273)

Write orders on your letterhead or prescription pad and have patient hand-carry to CCF.
Patient must register at A12 or S34 before proceeding to Lab (A15 or S15). Specimen Referral Testing Information.

Referral for Radiology Testing

Testing Information, 216/444-3082, 800/CCF-CARE (223-2273)

Write order on your letterhead or script pad and have patient hand-carry to CCF.
Patient must register before proceeding to X-ray. (EEG 216/444-6827, EMG 444-5559, MRI 445-7050)

Referral to Cleveland Clinic Florida

Appointments are made via Cleveland Clinic Florida ON-CALL Nurses program at 800/359-5101, EXT 2854. Spanish-speaking operators are available.
Foreign patient referrals can be facilitated by the International Services Department at 305/978-5080.
