- Gather all of your child's health information
you already have at home. Include health reports from all physicians,
hospital discharge summaries, school plan (IEP), test results,
and any other educational material you have received about your
child's diagnosis.
- Choose the pages you want to have in your Notebook
to help you organize your child's care more efficiently. You choose
the pages that will help you keep track of your child's care.
- Decide what information about your child is
the most important to keep in the All About Me Notebook
and consider storing the rest in a cabinet file drawer.
