Pre-registrations are accepted by fax or on-line until 4:00pm on December 11, 2007. To register after this date, you must register on-site. In case of a cancellation, a $25 fee will be deducted if cancelled by December 6, 2007. After December 6, 2007, a $50 cancellation fee will be deducted from your refund. Written notification of your cancellation is required in order to process your refund. NO REFUNDS WILL BE ISSUED AFTER DECEMBER 17, 2007.
Summit fee includes registration material, syllabus, continental breakfasts, refreshment breaks, luncheons on Friday and Saturday, Welcome Reception on Thursday and Dinner on Friday.
