Please complete the applicable Disclosure Form as completely as possible. It is important to provide appropriate detail to facilitate the review, analysis and protection of your invention. Signatures of all inventors and the Department/Division Chair are required at the time of submission. Once obtained, please scan and email an electronic copy of the disclosure to disclosure@ccf.org  to initiate Innovations’ efforts.
